I don't know. It rarely happens, but it does happen. All I then can see
in the log file is the IP address and the LMS version where the attempts
are coming from. And both don't really help me to understand what is
going on there. I've reviewed that code multiple time, with no outcome.
As you can see from the log file it's supposed to back off in case of
login failure, roughly duplicating the time before the next attempt on
every failure. Alas, it doesn't always work.
